From Out of Town and/or staying at the hotel:  
When scheduled You can check in at the hotel (Crown Plaza Knoxville) as early as 3pm the day
prior to your physical. If you are going to be later than 8pm contact the MEPS representatives at
the Hotel and let them know.

Check-in at the Crowne Plaza Knoxville (401 W SUMMIT HILL DR. KNOXVILLE, TN 37902) and
tell them you are a Navy Officer Applicant.  They will give you meal tickets for dinner and
breakfast.  You need to be at the MEPS no later than 6:30 a.m.  The MEPS is located at 9745
Parkside Drive, Knoxville, TN 37922.  Take I-40 West to N Cedar Bluff Rd Exit 378 (Left Lane).  
Turn Left at end of Ramp, then Right onto N Peters/Parkside Dr (You'll see a Famous Daves
Restaurant).   The MEPS is about a mile on right. (You can just follow the bus).

Directions From Hotel to MEPS  (I recommend you follow the Bus-  tell the bus driver you are
going to follow)

Note- MEPS policies: You must wear  full covering underwear. Women must wear a bra.
-Wear neat, moderate, comfortable clothing.
-Don't bring stereo headphones, watches, jewelry, excessive cash or any other valuables.
-Processing starts early at the MEPS - You must report on time- 6:30 am
-Dress in business casual, no flip flops or sandals.
-Leave Jewelry at home.
-Bring no weapons (Knives, Mace, blades, etc)
-Discuss any childhood medical problems and bring documentation with you.
-Have your Social Security Number (card not required) and driver's license.
-Remove earrings (they obstruct the headset used for the hearing test).
-Profanity and offensive wording or pictures on clothing is not tolerated.
-Hats are not permitted inside the MEPS.
-If you wear either eyeglasses or contacts, bring them along with your prescription and lens case.
-Bathe or shower the night before your examination.

You will have the following procedures at MEPS:
-a hearing and vision exam. Bring glasses and/or contacts (bring case and solution)
-blood pressure and pulse recorded.
-an HIV test
-a urinalysis (drug, protein, and sugars)
-an interview with a physician to record your medical history
-an orthopedic evaluation

Required Materials:
Applicants processing at Knoxville MEPS must provide  both Photo Identification (i.e. Picture ID
card) and proof of applicant SSN via any one of the below listed documents:

1. SSN Card (Original only. Copies not acceptable)
2. DD Form 214 (SSN)
3. DD Form 215 (SSN)
4. NGB Fm 22 (SSN)
5. Pay Stub/Statement (SSN)
6. Tax Deduction Form (SSN)
7.  W-2 Tax Statement (SSN)
8.  SSA NUMIDENT printout (SSN)
9.  Uniform Services Identification and Privilege Card (Photo & SSN)
10. Official School Transcript (SSN)

The MEPS will be required to capture a photo and fingerprints on EVERY applicant who presents
at the MEPS for processing.

If you sent us a FAX/EMAIL copy of the 680-3A-E and DD 2807-2, then be sure to hand-carry
the originals with you to MEPS.  If you gave us the originals, we will send them to MEPS before
your physical.
