Steps to apply for Medical Programs
First of all, it is important to realize there is NO OBLIGATION to apply to our programs. Even if
accepted to a program, you have the decision to accept or decline the position.  We recommend
applying to "scrolls" at the stage you are giving our programs consideration, to avoid any potential
delays.  The process to gain final selection may take 12 weeks or more.

Scrolls and Signed Resume/CV (Not required for NCP or HSCP).  Scrolls is
the process in which we submit your signed resume and a
Scrolls Request form to the Secretary of
Defense. Your name must be approved before you can get commissioned. You should send in
these documents to us when considering the Navy a slight career possibility.  This is a lengthy
process and we like to start this as soon as possible. Timely submission can make a difference of
thousands of dollars in your pocket when applying to HPSP student programs or Direct Accession
Programs.

Social Security Disclosure
-We need this form (Social Security Disclosure Statement) completed

-A photocopy of your Social Security Card

Medical Examination
Physical Examinations are conducted at Military Entrance Processing Stations (MEPS) or approved
DODMERB sites.  We will schedule the physical based on your availability. Basically, you tell us
what day works best for you.  MEPS starts physicals early in the morning and can last a few hours. -
If you do not live in the local area of a MEPS, we will pay for a hotel and meals during your stay -

You will need to complete two forms in order to take the physical:

- Request for Physical Examination 680-3A-E    PLEASE PRINT LEGIBLY
- Fill in Blocks B and D (don't know your selective service number?

- Fill in blocks 1-15, 21, 25-27, 28-29 (if applicable). In blocks 25-29, follow the directions. We need
a signature if the answer is "none"

- Report of Medical History 2807-2 (Initial all the applicable little blocks -"have you ever had ..."
instead of using an x).  If you answer YES to any of the conditions, you must explain in block "b" of
page three. Call us to  find out if any additional medical documents will be needed. Don't hesitate
to call us with questions.

N3M (Medical Review Section) POLICY:  Females must provide a copy of their last PAP exam (must
be less than one year old)

Application
-You can start working on your Application Processing and Summary Record at any time during this
process. Please type and be sure to save an electronic copy of your application.  Our processors
usually need to make corrections or fill in additional information.  Just fill it out to the best of your
ability. Once again, call us with questions. You can email it to us when you have completed as
much as you can.

Security Clearance
-Commissioned officers maintain a SECRET Security Clearance.  At some point along the way, we
will provide you access to a website for your security clearance application. You can use
this form
(SF-86) as a worksheet (not required),
but you will need to transcribe the data to
the online website once you get a login/password.

We will also need this form (NAVPERS 1070/613) completed for your security screening.

Official Transcripts
-You will need to order official college transcripts and have them sent to our office. We must have
official transcripts of all colleges/universities attended.  

-For Medical School, Dental School, and Schools of Osteopathic Medicine (HPSP & HSCP), we will
need an unofficial copy of AMCAS report (MD)/AACOMAS (DO) report, and unofficial copies of your
MCAT or DAT scores. You can usually print these directly from the AMCAS/AACOMAS websites with
your login.

Birth Certificate
-We need a copy of your birth certificate.

Interviews
-Some programs require interviews. We will arrange and schedule those if required. Appraisals
are placed on
these forms. (NAVCRUIT 1131/5).

Letters of Recommedation/References
-We often need 3 letters of reference/recommendation. These may be instructors, employers, or
personal.  You can be proactive and get them for us. Simple "To whom it may concern" letters of
1-3 paragraphs are fine. We use
this form (DD Form 370), but the letters can be in any format.  

Prior Service Records, if applicable
Prior Service?   We will need the following:

-Prior USN or USNR just need to fill out

Miscellaneous Forms
-Having us pay for your school with an HPSP program?   We just need this form (Academic Year
Statement) signed (we'll fill it out, but we need your signature.

-Depending on the program, we may need professional licenses, certifications, etc.

-Have a tattoo?  Complete these three forms:

-If applying for a direct accession program, tell us where you want to be stationed by completing a
Duty Preference Sheet (Navcruit 1131/16)

-If you are a physician applying to the Medical Corps Direct Accessions program, the two below
documents will help guide your credentialing package.

Understanding the Selection Process
Once we have compiled all required documents from your specific programs checklist, we send all
of the paperwork to Commander, Naval Recruiting Command for the
board process. This process
usually takes about 4-5 weeks to get results.

If selected, you will be eligible to be sworn in as an officer in the US Navy and embark upon the
training for your selected program.  For those entering into a non-student program, your orders to
Officer Development School (ODS) will be available PRIOR to your commissioning.
